All equipment must be returned to Singular Sleep within 10 days after receipt. I understand that if I do not comply with these terms and conditions, I will be subject to $15/day late fees and, with continued non-compliance, I may be subject to a replacement fee for the CPAP or BiPAP machine.
I agree to return the Auto-CPAP machine to Singular Sleep, LLC in the prepaid return packaging provided, with a postmark of no more than 10 days after the date I received it. I understand that I am responsible for the equipment until the prepaid, pre-labeled return box enters the mail circulation; if it is lost or stolen while in my possession, I am responsible for the full replacement value of $1000 for a CPAP machine and $2000 for a BiPAP machine. I agree to return the equipment intact and undamaged; I am responsible for the repair/replacement cost of any damaged/destroyed equipment.
I understand that after I submit my payment information for the order, my card will be charged $400 for the home CPAP titration service, which includes the use of the Auto-CPAP machine, the review of the titration data by a physician, the CPAP pressure recommendation, and the cost of USPS Priority Mail shipping both ways. The CPAP pressure recommendation can only be generated after the test has been returned to us; remote data interpretation is not possible. A $250 hold will also be placed on my credit card in the event that I fail to return the home sleep test within 10 days after it is delivered to me. I understand that at that time, if I have not placed the test back into the mail circulation, I will then be charged for the $250 hold that was placed on my credit card. If the machine is returned within 17 days from the $250 charge, I will be returned the $250 less late fees.
If the Auto-CPAP machine hasn't been received by Singular Sleep within 20 days after the hold is charged, my credit card will be charged the full replacement value of the machine minus $250. If the device is returned to Singular Sleep after the replacement fee has been charged, and the device is found to be in good working condition, the replacement fee will be refunded minus $15 per day additional late fees since the $250 hold was charged. Note: the replacement fee will only be refunded for up to 20 days after it was charged. After that, Singular Sleep must order replacement units so no refund will be given after that time.
If I return my testing unit to Singular Sleep missing any components (e.g. humidifier, air filter compartment, etc) I will immediately be charged for the replacement value of the component. If I return the component to Singular Sleep, the replacement charge will be refunded, minus $15 per day from the day the charge is incurred until the day Singular Sleep receives the returned item.
If I return the testing device to Singular Sleep with either no data or insufficient data for interpretation, I will have the option to either pay $50 for a second test to be sent to me, or receive a refund of the testing fee already paid, minus $50 for shipping and handling costs.
I understand that if I cancel my home CPAP titration order after it has already been shipped to me, I will receive a refund of the $350 fee, minus $50 for shipping and handling, for a total of $300. No late fees will be refunded, even if the test was unused.
