I agree to return the home sleep testing equipment to Singular Sleep, LLC in the prepaid return packaging provided, with a postmark of no more than two days after the date I received it. I understand that I am responsible for the equipment until the prepaid, pre-labeled return box enters the mail circulation.
I understand that when I submit my payment information for the order, my card will be charged $249 for the home sleep testing service, which includes the use of the testing device, the scoring of my test data by a physician, the test report that Singular Sleep provides to me, and the cost of Priority shipping both ways. A $500 hold will also be placed on my credit card in the event that I fail to return the home sleep test after 7 days of incurring late fees.
I agree that if I do not return the equipment within two days of receipt, I will begin incurring a $15 daily late fee for a period of 7 days. After 7 days of incurring late fees, I understand that I will then be charged for the $500 hold that was placed on my credit card. I also agree that if I do not return the laminated instructions that are included in the test, I will be charged a $15.00 replacement fee.
I understand that if I cancel my home sleep test order after it has already been shipped to me, I will receive a refund of the $249 fee, minus $30 for shipping and handling, for a total of $219. No late fees will be refunded, even if the test was unused.
If a test is 3 or more days late in being returned, no refunds will be given for an unused test or insufficient test data. Once the full replacement value of the device has been charged, the patient effectively owns the testing device. No refund of the replacement fee will be given, even if the test is later returned to Singular Sleep.
